When you submit the contact form, the name, email address, optional phone number, and message you provide are sent to Elisha by email and recorded in the website's connected Google spreadsheet. This information is used to review and respond to general or professional inquiries.
Do not submit clinical information
The contact form is not designed for HIPAA-secure clinical communication. Do not submit diagnoses, treatment records, insurance information, medication details, Social Security numbers, or emergency information. Submitting a form does not create a therapist-client relationship.
Interactive activities
Words, drawings, choices, and progress created inside the calming activities are used only in the current browser session and are not intentionally transmitted to or saved by KlatzkoTherapy.com.
Website operations and outside links
The site's hosting provider and normal internet infrastructure may process technical information needed to deliver and protect the website. Links and embedded media from outside organizations are governed by those organizations' privacy practices.
